WORK HOURS:  TuesdayFriday: 2-10pm and Saturday: 3-10pm

 

SUMMARY: 

The Friends For Life Corporation The Haven Outreach Specialist will serve as a primary person to provide recruitment, testing, and support to minority men, who are newly diagnosed with HIV/AIDS, or who have been out of HIV/AIDS medical care, and/or substance-using, and/or at high risk of contracting HIV and other STI’s. This position will also conduct outreach activities to community and collaborating partners and service providers.

ESSENTIAL DUTIES AND RESPONSIBILITES include the following:

Directly interacts with and provides support for HIV/AIDS infected men and their families. Performs HIV and STI counseling and testing services. Provides mentoring services to newly diagnosed individuals and persons lost to care. Provides risk reduction and adherence counseling. Assures linkage to care for newly diagnosed, out of care, and those at risk of leaving care. Assures retention in care for newly diagnosed, out of care, and those at risk of leaving care. Establishes a trusting relationship with HIV/AIDS infected/affected men. Initiates and maintains regular contact with assigned clients. Acts as an information resource to HIV/AIDS positive individuals. Acts as a liaison between the FFL Wellness University, Positive Living Center, Food Pantry, Housing, and other community based agencies and organizations. Maintains accurate, up-to-date written records of all client contacts. Maintains all client related information under strict confidentiality. Attends regular staff meetings. Maintain all files in accordance with funding source and agency standards. Make appropriate referrals for community resources for all persons supported. Make appropriate referrals for Friends For Life services for all persons supported. Track receipt of referrals for all persons supported. Provide accurate, complete and timely data entry and reporting of all relevant program and consumer data into CAREWare, HMIS, Evaluweb and/or other reporting systems as requested by supervisor. Other duties as assigned.

NON-ESSENTIAL JOB FUNCTIONS: Answer incoming telephone calls on an as needed basis. Performs light clerical work.

EDUCATION and/or EXPERIENCE: High school diploma or GED. Must have at least two years’ previous experience working with HIV/AIDS population. Must have experience with Microsoft Word.

CERTIFICATES, LICENSES, REGISTRATIONS: Must have a valid State Driver’s License, provide proof of automobile insurance coverage to employer at all times and have an automobile to be used daily to perform job duties.
